fix: Update skills matrix proficiency to 100% for all completed courses

- Set AI Fluency & Foundations to 100% (completed)
- Set Claude API Development to 100% (completed)
- Set Claude Code CLI to 100% (completed)
- Model Context Protocol already at 100% (both courses completed)
- Updated skill names to match actual course titles
- Added more accurate application descriptions

All completed certifications now correctly reflect 100% proficiency in their respective skill areas.
